Watchers/Risk Mitigation

Ad Hoc → Risk Mitigation Form (New, Update, or Discontinue)
–Expected that you are to do this at bedside
–Involve RN, Charge RN, RT, Family
–Reassess patient, update documentation
–Notify attending -  FYI primary attending (daytime) or Admitting Hospitalist (night time) if patient is made a Watcher or sending patient to PICU

PICU Response Team

  • No beeps. PICU to arrive within 15 minutes
  • Debrief with team members (RN, RT, Attendings) after event
    • RN fills out Debrief Form
    • Form is reviewed at monthly Pre-Arrest Prevention Meeting

Code White

  • Code Team Members expected to use role stickers
  • Bagging or compressions is considered Code White
  • Use if you need PICU at bedside immediately or earlier than 15 minutes (even if not bagging/compressions)

ED Response

  • ED responds to these codes
  • Do not need to attend, but if related to your patient, may be good to know what is going on to provide support to the patient/family
